ELDAC

ELDAC (End of Life Directons for Aged Care) is calling for expressions of interest from aged care service providers who wish to develop palliative care and advance care planning activities within their organisation. If you are selected for participation, your organisation may be eligible to receive an administrative grant of up to $20,000 to support your engagement in the program activities. Support at Home and Aged Care Act Transition Support Grant

The Support at Home and new Aged Care Act Transition Support 2024-25 Grant Opportunity (GO7421) is now open.

This grant will provide one-off funding of $10,000 (excl GST) towards the cost of upgrading IT systems for:

• Home Care Package providers to meet new obligations from 1 July 2025, under Support at Home and the new Aged Care Act.
• Commonwealth Home Support Programme providers to meet new reporting obligations under the new Aged Care Act and their 2025-27 grant agreements.
